Trinity Care Center was the largest nursing facility in Williamson County during the first quarter of 2026, data from the Centers for Medicare and Medicaid Services (CMS) shows.

The facility offered 179 beds—59 more than the county’s average for nursing homes in the same period—and assisted an average of 145.3 residents per day.

CMS gave Trinity Care Center an overall rating of 1 out of 5, which is below the Texas nursing home average of 2.7.

The Wesleyan Skilled Nursing and Rehabilitation and San Gabriel Rehabilitation and Care Center ranked as the second largest nursing homes in Williamson County for the quarter, each providing 142 beds.

As of the first quarter of 2026, Trinity Care Center had paid $10,293 in fines and received one penalty.

There were 11 for-profit nursing homes operating in Williamson County, Texas.

Estimates from the U.S. Census Bureau indicate that the population of older adults in the United States continues to rise as numbers for younger groups decrease. Between 2020 and 2024, the population aged 65 and older increased by 13%, while those under 18 dropped by 1.7%. The older adult population has grown significantly in the past 100 years—from 4.9 million in 1920 to over 61 million by 2024.

Research published in 2021 estimates that nearly 50% of this demographic, or about 28 million people, may need long-term services and support, with projections indicating more than a third could eventually require nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an organization focused on insurance education. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
